As a reminder, it is crucial that you keep your Tufts Health Plan records up-to-date with all active members' current and accurate mailing addresses and other contact information. Subscriber records should be validated and updated at least annually.
Up-to-date contact information ensures that subscribers to your company's plan receive all important mailings and communications, particularly the Form MA 1099-HC. The 1099-HC form will be mailed in January, 2009 to all subscribers. Members must receive this information in order to file state income taxes.
