Home Infusion as an Alternative Site of Care
As an alternative to hospitalization, Tufts Health Plan is offering a program with Coram Healthcare, a contracted provider of home infusion services, to provide intravenous antibiotics at home to members with cellulitis, pneumonia and other uncomplicated infections.
These services are covered as a supplemental benefit for Tufts Health Plan Medicare Preferred members and as a standard benefit for commercial HMO, POS and PPO members.
To be considered for the Alternative Site of Care Program, members must meet the following criteria:
- Be clinically stable
- Have a predictable course of therapy
- Have venous access
- Be willing to learn infusion therapy, or have a caregiver willing to do so
- Have a safe home environment
Since the beginning of the year, Coram has been meeting with network emergency rooms and physicians to review their protocols and program capabilities. Coram staff is available within four hours from the completion of the referral or request for services to dispense and administer the initial monitored dose and will collaborate with the primary care provider during the member’s ongoing care.
